The revised scope of work proposed replacing the Bravo Boot Part <br /> No.BFH-3-30-K-CTC. This revised scope of work for permit SR0055637 has been approved <br /> with the following conditions. <br /> Conditions of approval: <br /> 1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work.All new addenda supercede <br /> the previous ones. <br /> 3. All components must be certified by an independent testing organization. (UL,ULC, <br /> ANSI,API,ASTM, etc. See California Code of Regulations(CCR),Title 23 Chapter 16 <br /> Table B for complete list). The component must bear the appropriate mark as required by <br /> the independent testing organization. <br /> 4. If concrete is broken near the UST,a tank integrity test and/or line integrity test(or <br /> equivalent)must be performed, once concrete is poured in accordance with the <br /> manufacturer's guidelines. Results must be received by EHD within seven days of <br /> returning the facility to operating condition. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br />
