12. Submit the Best Management Practices to EHD for managing fuel spills and wash down water at <br />this site prior to the final inspection. <br />13. A "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br />submitted by the owner or the owners' agent [pursuant to CCR Section 2635(d)] before this install <br />will be considered complete by F -HD. <br />14. No gasoline, diesel, or other hazardous material or waste can enter the sewer/septic system. If it <br />does, it will be considered illegal hazardous waste disposal. <br />15. An accurate and drawn -to scale -as -built plant must be submitted to EHD prior to scheduling a final <br />inspection. <br />16. All laboratory results for the samples must be forwarded to this office directly from the lab. <br />17. Any hazardouswaste removed from the site must be hauled under a manifest and a copy of the <br />manifest must be forwarded to this office. <br />18. All personnel involved in the removal of the existing pipes must fulfill requirements of CCR title 8 <br />Section 5192. <br />19. Contact Muniappa Naidu, REHS at (209) 468-3439 for the following inspections; <br />A. Piping removal and sampling <br />13. Primary piping including product piping, vapor recovery and vent line piping soap and <br />pressure test. <br />C. Secondary piping including product piping, vapor recovery and vent line piping soap <br />and pressure test. <br />D. 24-hour secondary containment lake test on the sump. <br />E. 'Visual inspection of piping to insure proper slope prior to covering. <br />F. Spill bucket test for 5 gallon capacity. <br />G. Prior to receiving fuel, a complete functional test of tank and piping monitoring <br />systern(s) including overfill prevention. <br />H. Final inspection and demonstration of automatic shut down (if applicable). <br />20. This permit expires 90 days from the approval date.
