The following is a Minor-Notice to Comply <br /> violation: 109, 109/305, 111/311 <br /> 109. The a test report of the annual monitoring system certification, leak detector testing, and spill <br /> container testing performed on April 17, 2013 was submitted to the EHD on May 21, 2013, and a <br /> duplicate of the same report on May 22, 2013. A copy of the test report must be submitted within 30 <br /> days of the tests. Immediately begin submitting a copy of the report of test within 30 days to the EHD. <br /> 109 & 305. The UDC sump sensors were not tested due to lack of access, therefore continuous <br /> monitoring of the tank system could not be verified. These tests are required once every 12 months, <br /> that all monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br /> of product at the dispenser when it detects a leak. Immediately schedule these tests and provide 48 <br /> hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current <br /> hourly rate. <br /> 111 & 311. The line leak detectors were not tested due to lack of access. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi, and tests are required once every 12 months. <br /> Immediately have a properly licensed, trained, and certified contractor test the line leak detectors and <br /> notify EHD at least 48 hours in advance by April 30, 2014. Please be aware that a follow up inspection <br /> will be charged at our current hourly rate. <br /> NOTES <br /> The previously financial Responsibility and Chief Financial Officer letter were dated April 17, 2013; <br /> therefore still in effect today during this inspection. A new version of these document were provided to <br /> EHD during this inspection dated April 16, 2014.
