San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax:(209) 468-3433 Web:www.sjgov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GREWALS GAS & LIQUOR* 4100 E FREMONT ST, STOCKTON April 06, 2017 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS 11,or MINOR Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to print out the alarm histories for T1, T2, and T3 on the panel printouts for the <br /> 4/6/2016, 5/20/2016, 6/6/2016, 7/25/2016, 8/16/2016, 9/28/2016, 10/3/2106, 11/28/2016, 1/11/2017, and 2/27/2017 <br /> designated operator monthly inspection reports. The date on the panel printout(1/13/2017) does not agree with the <br /> date on the January 11, 2017 designated operator monthly inspection report. There is no date on the panel printout <br /> for the 10/3/2016 designated operator monthly inspection report. For the designated operator monthly inspection <br /> reports for 6/6/2016, 7/25/2016, and 8/16/2016, the designated operator indicated "Yes"for"Each alarm for the <br /> previous month has been responded to appropriately."Since there were no alarms during this period, the <br /> designated operator should have indicated "NA" (not applicable). The panel printout for the designated operator's <br /> 8/16/2016 monthly inspection report is missing the L9 alarm history. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The diesel VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it <br /> passed. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br /> The 87-product direct bury spill container failed the one-hour hydrostatic test. All LISTS shall be equipped with a spill <br /> container that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into <br /> the primary container or provides a means to keep the container empty. Immediately contact a properly licensed, <br /> trained, and certified contractor to repair or install an approved spill container under permit and inspection of the <br /> EHD. This facility cannot recieve any 87-product fuel deliveries until the spill container has been repaired/replaced. <br /> (Current inventory for the 87-product underground storage tank is 9,121 gallons). If the 87-product spill container <br /> does not need to be replaced, the facility owner must may for the re-insepction of the spill container at the County's <br /> currently hourly rate of$139.00 per hour. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 6, 2017. <br /> The 87-product spill container did not hold a capacity of five gallons.
