Hwy 99 Acampo, CA 95220 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />detector shutdown alarm and gross line failures on 1-30-12, 12-16-11, 7-24-11, and 7-23-11. During the <br />monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br />with documentation taken in response to any alarms to the monthly report. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />302 & 308. The test boots have been left on in the 1/2, 3/4, 5/6, 7/8, 9/10, and 11/12 dispenser sumps. <br />Any potential release from the primary containment into the secondary containment in the 87, 91, and <br />diesel piping does not flow into a collection sump. All secondary containment systems shall be sloped so <br />that all releases will flow to a collection sump located at the low point of the underground piping. <br />Immediately contact a properly licensed, trained, and certified contractor to ensure that any releases in <br />the 87, 91, and diesel piping flows into the dispenser sumps. <br />303. The diesel sump sensor was raised approximately 6 inches off the lowest point of the sump and not <br />located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able <br />to detect a leak at the earliest possible opportunity. During the inspection the technician relocated the <br />sensor to the lowest point of the sump. Continue to ensure that sensors are properly located at the <br />lowest point of the sump to detect a leak at the earliest opportunity. <br />306. A diesel fuel leak was found at the diesel filters in the 1/2, 3/4, 5/6, 7/8, and 9/10 dispenser sumps. <br />All primary containment must be product tight. Immediately discontinue deposition of diesel fuel into the <br />tank and have a properly licensed, trained, and certified contractor replace or repair the leak. Depending <br />upon the scope of work, the work may need to be performed under permit and inspection of the EHD. If <br />the failed component can't be repaired or replaced immediately, there is a possibility that the diesel UST <br />system may be red tagged to prevent fuel inputs. <br />310. Approximately one quart of liquid was found in the diesel turbine sump. Approximately one gallon of <br />liquid was found in the 91 turbine sump. If water could enter into the secondary containment by <br />precipitation or infiltration, it must be removed and disposed of properly. During the inspection, the <br />technician removed the liquid from the sumps. Continue to ensure that the sumps are maintained free of <br />liquid. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 7-14-12. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Aris Cacapit (209) 468-3453 <br />Received By: <br />vlow/ <br />Title: <br />EHD 23-02-003 Rev 04/19/12 1 CONTINUATION FORM <br />
