0 <br />July 3, 2012 <br />To <br />Aris Cacapit <br />EHD <br />Regarding Violation and Inspection dated 06111 /2012,The following has been done. <br />108. Secondary Containment test was done on time on 06/09/2011 but Afforda Test did <br />not Submit the Test Results onTime, from this time on they will be sending any and all <br />Test report on Time, same violation was sited in last year Report and was given the <br />response. <br />109. Monitoring inspection was done on time on 06/14/2012 but Afforda Test did not <br />submit the test results on time, they have been notified regarding this. <br />Inspection Results for June 11,2013 have been already submitted to EHD by me via Fax <br />and also by Afforda Test. <br />202 it has been corrected same day of Inspection. <br />203 and 306. Diesel Fuel was found leaking by the falter on pump 9/10 and 11/12.1 have <br />contacted Elite 4 contractors to come and replace: the Gasket on these Pumps and will <br />provide you with the Paper work with this report, Gasket ware change on July 2,2013. <br />303. Sump sensor for Diesel was found on opposite side and was corrected in front of <br />Aris Cacapit on the day of inspection. <br />310.
