Turner Road, LodiV, <br />I have reviewed the underground storage tank (UST) removal application for th j)�Lpve referenced <br />facility. The following items need to be addressed before the permit can be 4&1`67ed. <br />1. On page "1302" of the blue prints it indicates that "Repair (E) vent secondary containment <br />piping leak, field verify actual location". If a repair is to be made to the UST system then you <br />need to submit a repair application with the required fee to this office(enclosed). <br />2. Please complete and submit the enclosed contractor questionnaire. <br />3. You need to complete item number eleven (c) on page five of the application. <br />4.
