FACILITIES <br />To Whom it may Concern: <br />We respectfully request you update your records to reflect the following administrative <br />changes to our previously submitted Designated Operator information. We have enclosed <br />an updated submission form for each site within your jurisdiction highlighting the primary <br />Designated Operator for that facility. Additionally, you will find an addendum listing the <br />alternate Designated Operators.
