The removal of above -ground structures, equipment, and surface materials related to the UST <br />conversion will be performed in Compliance with all State and Local Regulations. <br />Appropriate action will be taken to avoid the spread of dust to occupied portions of the surrounding <br />area. Water will be used for dust control in such a manner as to not result in a hazardous or <br />objectionable condition. Compliance will be in accordance with the Air Pollution Control District <br />governing this area. <br />At this site with tank bury exceeding 5 feet, we will slope and bench excavation with accordance to <br />OSHA regulations. <br />During excavation to the top of the UST's, it is anticipated that no persons will enter an open <br />excavation. If it is deemed necessary, appropriate measures will be taken to protect personnel when <br />entering any excavation below five (5) feet in depth. These measures will include sloping or shoring <br />the walls of the excavation to protect individuals from hazards associated with cave-ins per OSHA 29 <br />CFR 1920. Trenching will comply with 8 CFR 1539 to 1547. NO INDIVIDUAL is to enter an <br />excavation until approved to do so by the SSHO. <br />Prior to the excavation activities, we will insure that buried gas lines, water lines, electric cables, and <br />other under underground utilities have been located by calling Underground Service Alert (USA). The <br />project site will remain fenced until the excavation(s) have been backfilled and is no longer a safety <br />risk. The SSHO will insure that unauthorized persons do not enter the site, and that all personnel on- <br />site have proof of up to date OSHA 29 CFR 1910.120 Training. <br />All personnel on-site will be required to wear Personal Protective Equipment as described in Section <br />7.3 of this HASP. A fire extinguisher will be placed in open view on site. <br />UTILITIES: <br />All related and non -related utilities above and below ground in the work area will be located by <br />Underground Service Alert (USA), and visual search methods. If possible, the utilities directly related <br />will be shut-off and disconnected prior to beginning the work. <br />PAVE ENT: <br />Concrete and asphalt will be removed by breaking it with a backhoe, bobcat, or jackhammer <br />equipped with a pneumatic attachment, and loaded into a dump truck for proper recycling and/or <br />disposal, or stockpiled on site for future removal. <br />11/07/2005 revision 5 4 <br />
