HEALTH & F T: 0 0 2007 <br />PERSONAL PROTECTIVE EQUIPMENT FAILURE: <br />If any site worker experiences a failure or alteration of protective equipment that effects the <br />protection factor, that person shall immediately stop that particular work activity and repair or <br />replace the failure. Should the failure of the personal protection equipment be unrepairable, <br />and then that worker will stop that work activity and report the failure to the SSHO. That site <br />worker will be reassigned to other work activity not requiring that particular PPE and shall not <br />be permitted to allowed to continue the previous work activity until the equipment has been <br />repaired or replaced. <br />• • • <br />If any other equipment on site fails to operate properly, the SSHO shall be notified and <br />determine the effect of this failure on continuing operations on site. If the failure affects the <br />safety of personnel or prevents completion of the work Plan Tasks, all personnel shall leave <br />the Exclusion Zone until the situation is evaluated and appropriate actions taken. <br />The SSHO shall submit an Incident Report to the Project Manager. This Report shall include <br />the following: <br />* Chronological history of the incident; <br />* Facts about the incident and when they became available; <br />* Titles and names of personnel; <br />* Actions: Decisions made and by whom; Order given to whom, and by <br />whom; and when; Action taken, who did what, when, where, and how; <br />* Possible exposure of site personnel; <br />* Report all injuries or illnesses during or as a result of the emergency. <br />These procedures are provided in addition to the HASP, prepared by Tait Environmental Systems <br />located at 1863 No. Neville St, Orange, CA 92865. <br />* Notify the appropriate personnel prior to the start of work. <br />* Notify the appropriate Jurisdictional Agencies prior to the start of work. <br />11/07/2005 revision 5 15 <br />
