H & SAFETY <br />and storage of hazardous materials according to Tait's existing practices and <br />procedures. <br />Tait shall have the appropriate fire extinguisher equipment on the job prior to the start <br />of the project. No cutting torch, open flame, or spark producing equipment shall be <br />used on the tank or piping at any time. Any source of ignition shall be at least 50 feet <br />away from the excavation site. <br />A properly calibrated flammable/combustible gas and oxygen analyzer is required on <br />the job site prior to excavation or removal. Calibration of the meter must be within the <br />last three months. <br />All electrical lines, piping, and like items known to be associated with the tank shall be <br />isolated or disconnected. <br />If at anytime during the excavation, the lower explosive limit of the atmosphere around <br />the tank exceeds 25 percent, all work will cease and proper mitigation procedures <br />initiated. <br />DO NOT ENTER TANK SUMP (S) FOR ANY REASON. Appropriate control <br />measures, such as fencing and confined space supervision, must be <br />employed prior to any worker entering into the tanks sumps. <br />All tank sumps are considered permit required confined spaces until they are <br />removed, and after the new sumps are installed and the concrete tank slab <br />has been poured. Monitoring and all other confined space procedures must <br />be followed when entering the sumps. THERE ARE NO EXCEPTIONS <br />Any and all soil removal and disposal is the sole responsibility of the client, and all soil samples <br />will be analyzed per clients' requirements. <br />11. Lock -out / Tag -out procedures (Energy Isolation) <br />Lock -out procedures must be followed during the cleaning, servicing, or adjusting of <br />machinery or electrical equipment. LOTO procedures shall be implemented as required <br />by applicable Federal, State, or Property Owner rules and regulations. <br />11/07/2005 revision 5 17 <br />
