A written monitoring response plan according to section 2632 <br /> (d) (1) & (2) of the California Underground Storage Tank <br /> Regulations must be submitted to PHS-EHD prior to opening the <br /> station. <br /> 2. The flex boot connector is only approved to be installed under <br /> the dispensers, and must be approved by a nationally <br /> recognized independent testing organization. <br /> 3 . The applicant must sign page three of the installation <br /> application. <br /> 4. All appointments shall be made 48 hours in advance. <br /> 5.
