Box 1836 <br />2600 Williams St <br />San Leandro, Ca 94577 <br />RE: Removal of Waste oil and temporary closure for gas two fuel tanks at 880 Victor <br />Rd, Lodi, Ca. <br />Public Health Services -Environmental Health Division (PHS-EHD) reviewed the <br />application your company submitted to this department for the removal of three tanks, <br />and determined that the site is not eligible for another removal permit. Public Health <br />Services -Environmental Health Division (PHS-EHD) is returning the application and the <br />check to your company. <br />In order for this facility to be in compliance with underground storage tank (UST) state <br />law and regulations, a temporary closure permit has to be obtained from Public Health <br />Services -Environmental Health Division (PHS-EHD). When the temporary closure <br />permit expires, the UST system must be upgraded or a closure permit application must be <br />submitted to remove tanks.
