Please <br /> send the following documents to the EHD to complete the application.The permit application is <br /> not approved at this time. <br /> 1. In the scope of work,indicate which penetration fittings are being replaced.Indicate if <br /> all product pipe and conduit penetration fittings are being replaced in the tank sksnps. <br /> 2. On page three,section four of the permil application,write in brand names and part <br /> numbers for all UST system pads being installed including penetration fittings,sump <br /> reducers,sump lids,spill containers.flex connectors,fiberglass product pipe and drop <br /> tubes. <br /> 3. Provide third party approval letters for all UST system pads being installed including <br /> penetration fittings,sump reducers,sump lids;spill containers,flex core=dors, <br /> fiberglass proclud pipe and drop tubes. Cut sheets which state"third party Approved' <br /> are not approval letters. <br /> 4. Provide a copy of the tank charts. <br /> 5.
