The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br /> certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a <br /> permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, <br /> there is a possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br /> 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. The service technician purged the lines free of air and <br /> retested, and the leak detector passed the annual test. <br /> 23d. The designated operator employee training log for all employees was not available for review. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is <br /> not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by February 12, 2010. <br /> Note <br /> 1.
