Page 4, Applicant signature in two places. <br />2. Page 5, Submit documents for #9, #10 and #11. <br />3. Page 7, #14b, The tank, piping and dispenser sensors are not listed in LG -113. <br />4. Page 7, #14c, Submit monitoring and response plan. <br />5. Page 7, #16a, Indicate that a deadman will be used. <br />6. Page 8, #18, Leak detection system not defined. <br />7. Page 9, See #23 and #26. <br />8. Page 9, Signature of acknowledgement. <br />9. According to the list of equipment to be used, it is indicated that an FE Petro <br />STP -MLD will be used. However, this leak detector is not compatible with <br />flexible piping. <br />10. Training records must be submitted. <br />11. Ball float percentage setting not indicated. <br />12. Buoyancy calculations from a professional engineer or a licensed architect <br />must be submitted. <br />13. There is a discrepancy in the under dispenser containment. According to the <br />plans, shallow pans will be used. Whereas, the list of equipment states a deep <br />pan. <br />If you have any questions please contact Dennis H.
