Monitoring reCprds must irxludo: <br /> 10, The UST opexato's name and telephone number; <br /> > A list orft types,quantities,and concentrations ofhazardous substances released; <br /> > A description of the actions taken to control and clean up the release; <br /> > The method and location of disposal of the released hazardous substances,and whether a hazardous waste manifest was or will be cued: <br /> > A description of actions taken to repair the UST and to ptevent fhturo releases; <br /> > A description of the method used to reactivate interstitial monitoring alter replacement or repair of primary containment• <br /> Reportable Releases: Any overfill,spill,or unauthorized rckease which escapes from secondary containment(or primary containni trt if nn secondary containment <br /> exists),increases the hazard of fire or explosion,or cmkM.any deterioration of secondary containment,is a reportable relcsse.
