A copy of the UST operating permit and all attachments <br />and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br />This is a minor violation. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to conduct the monthly designated operator inspections since July 2016. During the <br />monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank <br />top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators <br />performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the <br />reports. <br />This is a repeat violation, Class Il. <br />210 <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The designated operator monthly inspection reports from July 2016 through June 2017 were not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate <br />and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD. <br />This is a minor violation. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training was not performed since 2014. The designated operator shall train <br />facility employees for which he or she is responsible in the proper operation and maintenance of the UST system <br />once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a minor violation. <br />318 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 87 -product line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br />detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor <br />repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day <br />from the EHD.
