The permit has <br />been approved with the following conditions: <br />1. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />2. At least one person who holds the International Code Council "UST <br />Installer/Retrofitter" certification must be onsite AT ALL TIMES when work is <br />performed. <br />3. Manufacturer's certification of training must be presented at time of appointment <br />before any testing can take place. <br />4. Contact Lori Luces at (209) 468-0341 to arrange the inspection appointment(s); a 48- <br />hour notice for inspection appointments is required. <br />5. Permit expires 90 days from the approval date.
