The split tank annular sensor was raised approximately 9 inches off the lowest point of the split tank annular space and not located to detect a leak at the earliest opportunity. The designated operator failed to check and note the correct test dates for the secondary containment test on the 4-5-13 designated operator monthly inspection report. The designated operator report lists the secondary containment test as 3-4-10 but it should be listed as 3-25-13. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 303. The split tank annular sensor was raised approximately 9 inches off the lowest point of the split tank annular space and not located to detect a leak at the earliest opportunity. The sensor wire was measured at 109 inches from the bottom of the sensor to the top of the riser. The length from the bottom of the annular split tank space to the top of the annular riser was measured at 118 inches. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. During the inspection, the technician adjusted the sensor wire and verified that the annular sensor reaches the bottom of the split tank annular space. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by April 30, 2014. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
