The split tank annular sensor was raised approximately 9 inches off the lowest point of the split <br />tank annular space and not located to detect a leak at the earliest opportunity. The designated operator failed to check and note the correct test dates for the secondary <br />containment test on the 4-5-13 designated operator monthly inspection report. The designated operator <br />report lists the secondary containment test as 3-4-10 but it should be listed as 3-25-13. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />303. The split tank annular sensor was raised approximately 9 inches off the lowest point of the split <br />tank annular space and not located to detect a leak at the earliest opportunity. The sensor wire was <br />measured at 109 inches from the bottom of the sensor to the top of the riser. The length from the bottom <br />of the annular split tank space to the top of the annular riser was measured at 118 inches. Monitoring <br />equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. During the <br />inspection, the technician adjusted the sensor wire and verified that the annular sensor reaches the <br />bottom of the split tank annular space. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by April 30, 2014. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector <br />Received By: <br />�� <br />� <br />Title: <br />EHD 23-02 Rev 04119/12 Page 3 UST INSPECTION REPORT <br />
