Fremont <br />Stockton, CA 94568 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace electrical conduit penetration boot and/or 3.5" angle <br />penetration of the regular gasoline STP sump on the underground storage tank (UST) system at <br />the above noted address. Applicant stated in a phone conversation with EHD that the 3.5" <br />angle penetration is for the vapor piping, and that no concrete will be broken for this scope of <br />work. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeffrey Wong at (209) 468-0335 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Verification of the location of the repaired electrical conduit penentration and / or 3.5" <br />penetration, and if the penetrations are less than 2 inches above the product <br />penetration, a 24-hour hydrostatic test (or equivalent) on regular gasoline T-1 STP <br />sump. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possessor work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
