Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />The permit application for the removal and replacement of a turbine sump boot at the above <br />referenced facility has been approved under the following conditions. <br />1. Notify local Air Pollution Control District, and Fire Department for any applicable <br />permit. <br />2. Install only the approved products stated in the permit applications. <br />3. Install all approved products in accordance with manufacturers instructions. <br />4. Keep a copy of manufacturers installation instructions for Inspector review. <br />5. Arrange an inspection with an Environmental Health Inspector, so that product may be <br />inspected for UL labeling and 24 hour lake test/ or equivalent prior to backfilling. <br />6. All metal components in contact with backfill shall be corrosion protected. <br />7. Immediately upon completion of the job, this site shall require a third party tank integrity <br />test, by a licensed tank tester, and results shall be submitted to this department within <br />seven days. <br />8.
