California & (#5450) Aw <br />The permit applications for the above mentioned sites have been reviewed and found to <br />be deficient for the following reasons. <br />1. Lack of detailed scope of work. <br />2. Proof of Veeder-Root certification for contractor conducting the work. <br />3. A copy of the site health & safety plan. <br />y. CtuF �-" J--� -L-&/- ; , <br />The above items are needed before these plans can be reviewed any further. <br />Some other things that will be required as a condition of the permits are: <br />1. Contact the Local Fire department, and San Joaquin Valley Air Pollution Control <br />District for applicable permits. <br />2. Provide a Site Health & Safety Plan For inspection review at the job site. <br />3. Have a certified tank tester perform a tank integrity test once the work has been <br />completed. Submit test results within 10 days of testing. <br />4. Should new drop tubes be installed, their installation shall be noted on the scope <br />of work and all overfill prevention must satisfy California Code of Regulations <br />2635(2). <br />5. Schedule all inspection appointments 48 hours in advance. <br />6. Spill buckets must pass a 30 -minute lake test, or equivalent. <br />7. Contact San Joaquin Valley Air Pollution Control District to ensure proper spill <br />bucket and drop tube compatibility. <br />8. Should new drop tubes be installed, they will need to be inspected by this <br />department for proper overfill levels. <br />9.
