Fremont St., Stockton, CA 95203 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to repair the penetration fittings in the 87 main STP sump, 87 <br /> manifolded STP sump, 91 fill sump, UDC 3/4, and UDC 7/8 on the underground storage tank <br /> (UST) system at the above noted address. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at(209)468-3440 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. 24-hour hydrostatic test (or equivalent) on the 87 main STP sump, 87 manifolded <br /> STP sump, 91 fill sump, UDC 3/4, and UDC 7/8 sump. <br /> 4. If concrete will be broken near the USTs or piping, a secondary containment test must <br /> be performed on the UST or piping once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Secondary containment testing for the 87 main STP sump, 87 manifolded STP sump, 91 <br /> fill sump, UDC 3/4, and UDC 7/8 are required six months after installation of the repair <br /> fittings. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 6. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br />
