UST Facility Operating Permit Application and UST Tank Information forms are not current in CERS. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. Complete a separate tank form for each tank. 201. The facility alarm log for the November 7, 2012, L1 87 turbine sump alarm was not found on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility alarm log on site and submit copies to the EHD by August 29, 2013. 204. The designated operator failed to document the alarm from the attached alarm history on the December 2012 designated operator monthly inspection report and failed to check that it was responded to appropriately. The missing alarm includes: L1 87 turbine sump sensor alarm on November 7, 2012. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 310. Liquid was observed in the 89 and 91 turbine sumps. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the secondary containment sumps are maintained free of liquid. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by August 29, 2013. Ensure that all required information has been entered in the online California Electronic Reporting System. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
