Lockeford St. Lodi, CA Inspection Jul 29, 2013 <br />Name: p Address: Date: <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />105. UST Facility Operating Permit Application and UST Tank Information forms are not current in <br />CERS. Any change of information must be updated in CERS within 30 days of the changes. <br />Immediately log into CERS, update the required information, and submit for review by the EHD. <br />Complete a separate tank form for each tank. <br />201. The facility alarm log for the November 7, 2012, L1 87 turbine sump alarm was not found on site. <br />These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility <br />alarm log on site and submit copies to the EHD by August 29, 2013. <br />204. The designated operator failed to document the alarm from the attached alarm history on the <br />December 2012 designated operator monthly inspection report and failed to check that it was responded <br />to appropriately. The missing alarm includes: L1 87 turbine sump sensor alarm on November 7, 2012. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />310. Liquid was observed in the 89 and 91 turbine sumps. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly.
