Item #106. Updated Monitoring & Response Plan is enclosed. <br />2. Item #110. Leak Detector issue addressed in email dated 10/9/12. <br />3. Item #113. Updated Owner Statement listing Designated Operators and <br />ICC Certifications is enclosed. <br />4. Item #204 & 206. Initial Employee Training for Ravi Singh was <br />conducted on 8/14/12, according to Walton Engineering, the D.O. for the <br />site. The record for that training is maintained on site. <br />5. Item #310. The liquid removed from STP and Fill Sumps was picked up <br />by Quik Stop's HW contractor on 9/18/12 and transported under Manifest <br />#005771824 FLE ("Designated Facility to Generator" copy is enclosed). <br />6.
