The monitoring plan was not current when field verified for accuracy <br /> during the monitoring system certification testing. The monitoring plan for each tank (87, 89, and 91) did not indicate <br /> the proper manufacturer's name for Tank and Piping monitoring equipment on site. The leak sensor manufacturer was <br /> listed as Gilbarco. It should be listed as Veeder Root. The monitoring plan must be uploaded to the California <br /> Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for <br /> review by the EHD. <br /> This is a minor violation. <br /> 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br /> Records of repairs for the UST system were not found on site. The L2 89 stp sump sensor had a fuel alarm on <br /> 10-27-14 and the L5 89 annular sensor had a fuel alarm on 10-28-14. The designated operator indicated that Elite IV <br /> responded to these alarms. Maintenance records for the response to theses alarms were not attached to the <br /> desiqnated operator monthly inspection reports. These records shall be kept on site for the life of the tank. <br /> Immediately locate and maintain these records on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Rupy Sanghera is not current and she was left to work alone at the <br /> store. The designated operator shall train facility employees for which he or she is responsible in the proper operation <br /> and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
