Lockeford St. Lodi, CA 95240 SR0071019 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to install a Bravo F-Series F-07 retrofit boot for the 87 stp piping <br /> sump electrical penetration and to install Bravo flex boots on the 87 and 91 secondary lines at <br /> the under dispenser containment sumps of the underground storage tank (UST) system at the <br /> above noted address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Veloso at (209)468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Secondary piping soap test: One hour pressure test per manufacturer's guidelines. If <br /> there are no manufacturer's guidelines, test shall be performed using an applicable <br /> method specified in an industry code or engineering standard. <br /> b. 24-hour hydrostatic test (or equivalent) on 87 stp piping sump. <br /> c. Secondary containment testing on 87 and 91 secondary lines. <br /> 4. If concrete will be broken near the UST and/or piping, a secondary containment test <br /> must be performed on the affected UST and/or piping once concrete is poured in <br /> accordance with the manufacturer's guidelines. <br /> 5. Secondary containment testing for the 87 and 91 secondary lines and the 87 stp piping <br /> sump are required six months after installation. Provide 48 hours notification to the EHD <br /> when this test is scheduled. <br /> 6. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br />
