The monitoring plan for all three tanks indicates <br /> "Yes"for Tank Integrity Testing Results. Change this to"No". The monitoring plan must be uploaded to the California <br /> Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for <br /> review by the EHD. <br /> This is a minor violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. Add an "X"to the annular space to indicate that the space <br /> is monitored by a sensor. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/and upload a <br /> copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br /> Secondary containment testing was performed on December 16, 2015 and a test report was submitted on January 26, <br /> 2016, approximately ten days late. A copy of the test report must be submitted within 30 days of the tests. Take all <br /> necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. <br /> This is a minor violation. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on June 16, <br /> 2015 and a test report was submitted on August 12, 2015, approximately one month late. A copy of the test report <br /> must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test <br /> reports are performed in a timely manner. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator marked N/A(not applicable)for"Has each alarm for the previous month been responded to <br /> appropriately" on the January 22, 2016 designated operator monthly inspection report. This was not appropriate since <br /> there was a fuel alarm on January 17, 2016 for L1 at dispenser 1-2 that the designated operator did address in the <br /> January 22, 2016 and February 26, 2016 designated operator monthly inspection report. Also, the designated <br /> operator addressed the February 18, 2016 high product alarm for T1 (product 87) in the March 30, 2016 designated <br /> operator monthly report instead of the February 26, 2016 designated operator monthly report. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> Page 4of5 <br /> I <br />
