The 91 direct -bury spill container failed when tested. The water level stopped 1/4 capacity above <br />the drain valve and did not completely drain. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />discontinue deposition of petroleum into this tank until the component is repaired or replaced by a <br />properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the spill <br />container can't be replaced immediately, there is a possibility that the 91 UST system may be red tagged <br />to prevent fuel inputs. <br />Submit to SJC EHD within 30 days (08/16/13): <br />1. permit to repair the 91 direct bury spill bucket. No fuel deposition until repaired. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD 23-02 Rev 04/19/12 v Z Page 3 UST INSPECTION REPORT <br />
