The designated operator failed to check and note the correct test dates for the secondary <br />containment testing on the designated operator monthly inspection reports from September 2010 to <br />present. The designated operator has written the date as 12/11/10 rather than 12/11/09. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />Submit to SJC EHD within 30 days (08/12/11): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for the above violation <br />Notes: <br />- Last secondary containment test performed 12/11/09. Next tri -annual testing due 12/2012. <br />- Provided free training schedule July - December 2011.
