Responses to the violations, discussed in the order listed in <br />the inspection report, follows: <br />Pressurized piping integrity test results are enclosed for 1993. In 1993, <br />automatic turbine shutoff was installed, eliminating the requirement to test <br />lines annually. <br />2. Pipeline mechanical leak detector tests were performed as part of the piping <br />integrity testing in 1993, and as part of the monitoring system certification in <br />1994. Results are enclosed. <br />3. Annual monitoring system inspection and certification for 1994 is enclosed. <br />Apparently, my predecessor did not require written documentation from our <br />maintenance contractor certifying proper operation. Therefore, 1993 annual <br />monitoring system certification could not be located in our files. <br />4. The broken spill bucket fuel release lever was reported to Quik Stop's <br />authorized maintenance contractor, Elite IV Contractors, Inc., on 5/26/94. <br />
