Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />The permit application for the replacement of a spill bucket at the above mentioned site has been <br />approved with the following conditions. <br />1. <br />Contact local Air & Fire districts for required permits. <br />2. <br />A third party certified tank tester must conduct a tank integrity test Immediately <br />following completion of the job and submit results directly to this department within <br />seven days. Should the tank Integrity test require follow up time, that time shall be billed <br />to this permit at the EHD hourly rate of $93.00 per hour. Should the tank be brine filled <br />the tank integrity test may be waived if the tank, annular sensors are inspected by this <br />department at the completion of the job. The tank integrity test may also be substituted <br />with a 0.1 GPH tank test if the monitoring system has this capability. <br />3. <br />Overfill prevention must satisfy Title 23 regulations for overfill. Section 2635(b)(2). <br />4. <br />Overfill drop tubes shall be re -certified for length at the time of the final inspection. <br />5. <br />All overfill buckets shall hold no less than 5 gallons. <br />6. <br />All equipment shall pass a 30- minute hydrostatic lake test in the presence of an EHD <br />Inspector. <br />7. <br />The contractor conducting the job shall be certified by the equipment manufacturer prior <br />to installation. <br />8. <br />All installations shall follow manufacturers recommendations. <br />9. <br />Arrange all inspections 48 hours in advance. <br />10. <br />This permit expires in 90 days. <br />11. <br />Keep a copy of this permit, and manufacturers installation recommendations on site for <br />inspector review. <br />12. <br />Should any of the newly installed equipment come in contact with backfill it shall require <br />and be inspected for corrosion protection. <br />For Questions or appointments call (209) 468-3440 <br />Thank you, <br />John Jackson REHS <br />
