The following <br /> are Class II (Repeat) : 104, 204, 311 <br /> 104. Financial responsibility documents were not submitted to the EHD nor uploaded to CERS. Current <br /> financial responsibility documents are required to be submitted annually. Immediately submit a copy of <br /> the financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required <br /> information, and submit for review by the EHD. Received a current copy of the financial responsibility <br /> and Chief Financial Officer letter dated March 10, 2014 and April 21, 2014, respectively, during this <br /> inspection. Violation has been corrected on site. <br /> 204. The designated operator failed to note the correct test date (July 11, 2012) for the Secondary <br /> Containment Test on the last 12 (May 2013 to April 2014) designated operator monthly inspection <br /> reports, entering a date of August 3, 2011 instead. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> 204. The designated operator attached only a single slip of print-out stating "No Record Found" with no <br /> documentation that the alarm log was checked or the alarms were responded to appropriately on the <br /> June 2013 to February 2014 designated operator monthly inspection reports. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> 311. The 91-octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak <br /> detector, retested it, and it passed. <br /> Note: 91-octane SB tested after Elite IV replaced the drain valve.
