(Repeat) Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. A copy of the financial responsibility <br />has been submitted on site. Violation was corrected. <br />108. (Repeat) Secondary containment testing for 87 and diesel STP sumps were last performed on <br />1/23/2009 was last performed on and was due by 1/31/2012. (Secondary containment testing for 91 <br />STP sump was last performed on 7/27/10, and secondary containment testing for the pipe -lines, USTs, <br />and UDC sumps were last performed on 3/12/2012 and 3/16/2012.) Testing for 87 and diesel STP <br />sumps has not been completed and are 5 months past due. Secondary containment testing is required <br />once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD, and <br />submit test report within 30 days of the test. <br />303. The wire of the 91 STP sump sensor was approximately one foot too short laterally for the sensor <br />to be located at the bottom of the STP sump to detect a leak at the earliest opportunity. Monitoring <br />equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Garod <br />Burke of Elite IV replaced the existing INCON TSP-ULS liquid sensor with a new sensor of the same <br />model, which provided sufficient length for the sensor to be located at the bottom of the STP sump to <br />detect a leak at the earliest opportunity. An in-house UST Retrofit Permit completed by inspector on site <br />has been filed documenting this repair. Violation was corrected. <br />NOTES <br />Garod Burke did not have his ICC service tech and Incon manufacturer training certification on site. In a <br />phone conversation with Kim of Elite IV during this inspection, she stated a copy of his ICC service tech <br />and Incon manufacturer training certification will be submitted to EHD by Monday next week (July 2, <br />2012.) <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />HIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Ef23-0211 Rev �3/19lP2� v Frage 3 UST INSPECTION REPORT <br />/ V Y. <br />
