Y ,,` a,,...: •i i N. .83 m it ..d.F' ..I..'-i — <br /> Item# Remarks <br /> 115 CCR 2715(a) Failed to submit to EHD notification of designated operator identification or change Win 30 days. <br /> A new designated operator, David Turner has signed DO reports since July 2017 and notification was not provided <br /> to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately <br /> log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the <br /> notification identifying all the designated operators for this facility. <br /> This is a repeat violation, Class 11. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to fully review the alarm history from the attached alarm history on the June 2017 <br /> and the January 2018 designated operator monthly inspection report and failed to check that they were responded <br /> to appropriately. The alarm history report for December 2017 was done on 12/20/2017 and the date range for the <br /> alarm history started on 12/26/2017 for the January 2018 DO repot, leaving over 5 days unaccounted for in the <br /> alarm history report. The alarm history report for May 2017 was done on 5/23/2017 and the date range for the alarm <br /> history started on 5/31/2017 for the June 2017 DO repot, leaving over 7 days unaccounted for in the alarm history <br /> report. Several months of the reports had a no alarm printout attached to them, but had no date range or history to <br /> determine which days were verified. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, and attach <br /> a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> This is a repeat violation, Class II. <br /> 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Heng Chea and Fong Chea is not current. The designated operator <br /> shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST <br /> system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class 11. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 91 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi.
