Any changes shall be reported to the EHD within 30 days. Immediately log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the <br /> designated operators for this facility. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The maintenance records alarm <br /> logs or records of follow up actions for"SN2 sensor on alarm" on 04/29/2016 were not found on site. These records <br /> shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit <br /> copies to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the May 25, 2016 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: "SN2 sensor on alarm"for Dispenser3/4, Dispenser 1/2, Diesel Annular, UNL Annular, <br /> Diesel Sump, and Prem Sump that occurred on 04/29/206. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a minor violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The audible alarm on the control panel failed to activate when tested to ensure continuous monitoring of the tank <br /> system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and <br /> certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable) and <br /> obtain a permit within one business day from the EHD.
