Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on 05/28/10 and was completed today, one calender month past due. The test <br /> should have been completed by end of 05/2011. These tests are required once every 12 months. Take <br /> all necessary precautions to ensure testing is performed in a timely manner. <br /> Submit to SJC EHD within 30 days (07/11/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violation <br /> 3. requested/supporting documentations <br /> Notes: <br /> - Certification of financial responsibility insurance coverage by Liberty Surplus Insurance Corp policy <br /> ends 07/07/2011 for all of Anabi Oil Corp UST facilities. In addition, coverage is only for 3 UST. There <br /> are 4 UST at this facility (diesel UST not on policy). Diesel UST is not covered at 3725 Tracy Blvd either. <br /> - A scheduled secondary containment testing was performed 05/20/11. Ensure test results are submitted <br /> to EHD by 06/20/11. <br /> - A new designated operator, Dave Thomas from Service Station System, performed the June monthly <br /> visual inspection report on 06/06/11. Ensure an Owner statement of Designated Operator notification <br /> listing all designated operator for this facility is submitted to SJC EHD within 30 days of the designated <br /> operator report. <br /> UST system notes- <br /> - Diesel LLD is wireless <br /> - Two STP per product, one LLD per STP. 'A' LLDs have test ports on UDC 1/2. 'B' LLD have test ports <br /> on UDC 17/18. <br /> - overfill prevention is ball floats on vapor risers. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSP C AT IME EH 'S CURRENT HOURLY RATE. <br /> EHD Inspector: Receiv B .
