The customer wants EPIC to update and file paperwork <br /> to ensure an updated Permit to Operate for all sites. <br /> Two (2) of those sites are under the jurisdiction of the San Joaquin Environmental Health <br /> Department(CUPA). <br /> I'd like to know what is needed. Is it a simple letter on letterhead informing the change of <br /> ownership, the site names and the site address? Or is it the full fledged paperwork with: <br /> 1- Business Identification Form <br /> 2- UST Form A and Form B <br /> 3- Certificate of Financial Responsibility <br /> 4- HMBP, Monitoring and Response Plan <br /> There have been no equipment changes and no site plan changes from what is <br /> already in existence. Also, they do not have much information as the previous owner <br /> (Shell Corporation) removed the paperwork for the Permit to Operate when they completed <br /> escrow. We have visited the sites to perform the DO Inspection for them, and have noted <br /> the removal of paperwork, including past results. <br /> The site list is attached for your reference. <br /> 8/2/2010 <br />
