The paperwork necessary for the Environmental Health
Department includes the UST Form A including their BOE number, Form B's, Financial Responsibility, Monitoring and Response
Plans, a permit transfer fee, plus a list of other forms that are included in our new consultation packet.
The owner has already paid for the consultation with the area inspector, Stacy Rivera, and been mailed a packet with all of the
required Environmental Health forms. The owner just needs to make an appointment with Stacy.
The San Joaquin County Office of Emergency Services also needs to be contacted about the change in ownership for their
Business Plan requirements. You can contact them at xsioes@co.san joaquin.ca.us or(209) 953-6200.
Thank you for your interest.
Kasey Foley, REHS
Program Coordinator
San Joaquin County Environmental Health Department
600 East Main Street
Stockton, CA 95202-3029
(209)468-3451
kfoley@sjcehd.com
From: Fadi Afa AI-Refaee [mailto:frefaee@epiccompliance.com]
Sent: Wednesday, July 21, 2010 10:05 AM
To: Kasey Foley [EH]
Cc: Donna Heran [EH]
Subject: Gas Station Change of Ownership: Sites under San Joaquin Environmental Health (CUPA) Oversight
Good day Kasey,
We are an environmental compliance business and have a customer who has acquired 26 sites in various CUPA
jurisdictions. The customer wants EPIC to update and file paperwork to ensure an updated Permit to Operate for all sites.
Two (2) of those sites are under the jurisdiction of the San Joaquin Environmental Health Department (CUPA).
I'd like to know what is needed. Is it a simple letter on letterhead informing the change of ownership, the site names and
the site address?Or is it the full fledged paperwork with:
1- Business Identification Form
2- UST Form A and Form B
3- Certificate of Financial Responsibility
4- HMBP, Monitoring and Response Plan
There have been no equipment changes and no site plan changes from what is already in existence. Also, they do not
have much information as the previous owner(Shell Corporation) removed the paperwork for the Permit to Operate
when they completed escrow. We have visited the sites to perform the DO Inspection for them, and have noted the
removal of paperwork, including past results.
The site list is attached for your reference.
7/21/2010
