Grant Line Rd., Tracy SR0060283 <br /> The San Joaquin County Environmental Health Department has been informed that you are the <br /> new owner or operator of the underground storage tanks (UST's) at the above-mentioned <br /> facilities. The forms listed below have been included with this packet. Please complete these <br /> forms for each facility and contact Stacy Rivera at (209) 468-3440 to arrange for an office <br /> consultation to review and submit these forms. These forms must be completed within 30 days <br /> of the close of escrow [CA Health and Safety Code section 25284(b) &(c)]: <br /> 1. Facility Form (UST-A): Provide all current information, sign and date at the bottom. <br /> 2. Tank Forms (UST-B): Provide all current information, sign and date the bottom of each <br /> form. Complete one form for each UST at the facility. <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer. Complete <br /> the forms provided, and make a copy for our records. These documents are required to <br /> be submitted to this office annually. <br /> 4. Monitoring (UST-D) and Response Plans (UST-E): Complete and submit a copy of each <br /> for approval. An approved copy will be returned. Retain an approved copy of these plans <br /> onsite for future reference by employees. <br /> 5. Statement of Designated UST Operator. If you are the owner, you must submit a <br /> notification form identifying the "Designated UST Operator' for each facility owned. A <br /> notification form is enclosed for your use. <br /> Complete the following items for each facility within 30 days of the close of escrow [CA Health <br /> and Safety Code section 25284(b)& (c)]: <br /> 6. Hazardous Materials Management Plan — Contact the Office of Emergency Services at <br /> (209) 953-6200 for information on updating this plan. <br /> 7. Board of Equalization UST Storage Fee Account Number — Call (916) 322-9669 to <br /> obtain a current number for each facility. <br /> 8. Operating Permit for Vapor Recovery System on the UST— Contact the San Joaquin <br /> Valley Air Pollution Control District at (209) 557-6400 for information on obtaining the <br /> required permits. <br />
