CLASS H. or MINOR - Notice to Comolv) <br />Item # <br />Remarks <br />106 <br />CCR 2632 Monitoring and response plans not current or approved by the EHD. <br />The monitoring plan is not current and/or not approved by the EHD. The submitted monitoring plan for the 87-octane tar <br />indicates Veeder Root monitoring panel as "TLS-350" in one section of the plan and as "TLS-350 Plus" in another <br />section, also visual daily pipeline monitoring was indicated for this suction system; please verify the tank monitoring anc <br />resubmit as appropriate. The submitted monitoring plan for the diesel tank indicates that annual tank integrity testing (23 <br />CCR 2643.1) as well as the visual daly pipeline monitoring is being conducted for this tank, please verify the tank <br />monitoring and resubmit as appropriate. The accurate monitoring plan must be uploaded to the California Environmenta <br />Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the <br />EHD. This is a Class II violation. <br />107 <br />CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />An accurate UST Monitoring Site Plan was not submitted. A new Veeder Root monitoring panel and new sensors were <br />installed in 2014, however, the submitted map still shows the preexisting Tank Guard monitoring components. An <br />accurate site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into <br />the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST <br />Monitoring Site Plan. This is a Class II violation. <br />110 <br />CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br />Secondary containment testing was performed on 12-8-2014 and a test report was submitted on 1-16-2015. A copy of <br />the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and <br />submittal of test reports are performed in a timely manner. This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by February 22, 2015. <br />Consent to perform the inspection, take photos and collect samples was given by Mike Edgar, Food Safety <br />Manager. This facility's designated operator is Zane Nimmo (ICC expiration date: March 3, 2016) . The service <br />technician was David Winkler] (ICC expiration date: March 3, 2016), who had the following manufacturer's <br />certifications: VR: February 20, 2016. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if <br />applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br />Compliance. <br />NOTE: The 87-octane annular sensor could not be removed from its riser for testing (was stuck), and as such, its <br />functionality could not be verified today. The last monitoring system certification was conducted on June 5, 2014, <br />and although the monitoring system certification testing was not due, the facility elected to reset their certification <br />date and conduct the pertinent testing today.
