Current approved copies of the monitoring plan was not found on site. The monitoring plan <br />must be current and approved by the EHD. Immediately log into CERS, update the monitoring plan <br />information, and submit for review by the EHD. Complete a monitoring plan record for each tank. <br />Maintain an approved copy on site. <br />108. According to the test results submitted to this office secondary containment testing was last done <br />on February 15, 2011. Test results were received by this office January 26, 2012 with a signature by the <br />tester dated December 15, 2011. Testing notification was received by this office on November 3, 2011 <br />with a test date of December 15, 2011. According to Michael Edgar he believes the test was done in <br />February 2011. It appears testing has not been completed and is 3 months past due. Secondary <br />containment testing is required once every 36 months. Immediately schedule this test and provide 48 <br />hours notification to the EHD. <br />109 & 111. Annual monitoring system certification, and spill container testing were last performed on <br />January 25, 2013 and was completed today, June 5, 2014, 4 months past due. These tests are required <br />once every 12 months. Take all necessary precautions to ensure testing is performed in a timely <br />manner. <br />204. The designated operator failed to check and note the correct test dates for the secondary <br />containment testing for the inspection dated December 5, 2013. It was noted as 12-15-12. Secondary <br />containment testing is noted as 12-15-11 on all other complete DO reports reviewed January 2013 -May <br />2014. DO reports 11-13,10-13, 8-13, and 5-13 do not have the second page attached and are <br />incomplete. During the monthly inspection, the designated operator shall check that all required testing <br />and maintenance for the UST system have been completed and document the dates they were done. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. Submit copies of the incomplete DO reports listed to this office <br />within 30 days (July 6, 2014). <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
