SUMMARY OF VIOLATIONS (CLASS I, CLASS 11, or MINOR -Notice to Comply) 106 & 202. Current approved copies of the monitoring plan was not found on site. The monitoring plan must be current and approved by the EHD. Immediately log into CERS, update the monitoring plan information, and submit for review by the EHD. Complete a monitoring plan record for each tank. Maintain an approved copy on site. 108. According to the test results submitted to this office secondary containment testing was last done on February 15, 2011. Test results were received by this office January 26, 2012 with a signature by the tester dated December 15, 2011. Testing notification was received by this office on November 3, 2011 with a test date of December 15, 2011. According to Michael Edgar he believes the test was done in February 2011. It appears testing has not been completed and is 3 months past due. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. 109 & 111. Annual monitoring system certification, and spill container testing were last performed on January 25, 2013 and was completed today, June 5, 2014, 4 months past due. These tests are required once every 12 months. Take all necessary precautions to ensure testing is performed in a timely manner. 204. The designated operator failed to check and note the correct test dates for the secondary containment testing for the inspection dated December 5, 2013. It was noted as 12-15-12. Secondary containment testing is noted as 12-15-11 on all other complete DO reports reviewed January 2013 -May 2014. DO reports 11-13,10-13, 8-13, and 5-13 do not have the second page attached and are incomplete. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit copies of the incomplete DO reports listed to this office within 30 days (July 6, 2014). ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
