Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Sensor/Monitoring System functionality: Functionality of the monitoring system <br />for all sensors must be verified to ensure that monitoring has been restored to <br />meet the requirements of Title 23 California Code of Regulations and the Health <br />& Safety Code. A copy of the alarm history shall be printed prior to performing <br />the "cold start" of the Veeder Root system and made available at the time of EHD <br />inspection. <br />4. Since concrete will be broken near the existing USTs (diesel and regular unleaded), a <br />---�� secondary containment test must be performed on the USTs once concrete is poured in <br />accordance with the manufacturer's guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
