UST facility form is lacking BOE Maintenance Fee Account Numbers or BOE number is incorrect. <br />Immediately complete and submit a copy of the facility form to the EHD. The new owner/operator is in <br />the process of getting a new 130E number <br />302 & 304. The 87 annular sensor failed to activate an audible and visual alarm when tested and, <br />therefore, the tank is not monitored. Also due to the 87 annular sensor in alarm the diesel annular <br />sensor could not be tested. The interstitial space shall be monitored with a monitoring system <br />connected to an audible and visual alarm system.
