UST facility form is lacking 130E Maintenance Fee Account Numbers or BOE number is incorrect. <br />Immediately complete and submit a copy of the facility form to the EHD. The new owner/operator is in <br />the process of getting a new BOE number <br />302 & 304. The 87 annular sensor failed to activate an audible and visual alarm when tested and, <br />therefore, the tank is not monitored. Also due to the 87 annular sensor in alarm the diesel annular <br />sensor could not be tested. The interstitial space shall be monitored with a monitoring system <br />connected to an audible and visual alarm system. Immediately contact a properly licensed, trained, and <br />certified contractor to replace the failed component with a functional component (LG 113-x listed, if <br />applicable) under permit and inspection of the EHD within one business day. <br />Notes <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation to the EHD within 30 days of receiving <br />this report. <br />Performed consultation today and received Facility form, Tank form, Certification of financial <br />responsibility, D.0 owner statement, Monitoring and Response plan (approved), and 5021 (change of <br />information form. <br />provided free class shedule <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />Inspector.
