EI Dorado St., Stockton <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />ROUTINE ANNUAL INSPECTION AND MONITORING SYSTEM CERTIFICATION <br />Violations: (13, 23d) <br />13. The sensor in the 87 piping sump failed to stop the flow of product at the dispenser when tested. <br />The audible and visual alarm was activated, but the relay for the 87 tank was stuck in the on position. <br />All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. The relay was unstuck by the technician on site. The <br />87 piping sensor was retested and passed, including positive shutdown. <br />23d. The designated operator employee training for Francisco Sison was not current. Mr. Sison was the <br />only employee on site and was last trained on February 25, 2010. The designated operator shall train <br />facility employees for which he or she is responsible in the proper operation and maintenance of the <br />UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by June 26, 2011. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by June 26, 2011 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTI NY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Ins edor: Recei ed <br />Title <br />EHD 23-02-003 -' Rev 08!10/10 `� CONTINUATION FORM <br />
