System Check Form, it was not logged on the UST System Alarm Response Log <br />onsite. Alarm logging procedures have been reviewed with store personnel and this <br />alarm has since been logged correctly. <br />The L28 alarm on March 11, 2013, is covered by Jones Covey work order #19581, <br />which was reviewed by the designated operator during the monthly inspection on <br />March 14, 2013, and included with the designated operator monthly inspection <br />report. This alarm was captured by the PFJ HelpDesk, the store was contacted, and <br />it was determined that a technician was onsite with an EHD Inspector. The <br />designated operator performed a visual inspection of this sump and noted that it was <br />free of water, debris and hazardous substances, as well as verifying that the sensor <br />was in the proper position. The alarm was logged by store personnel, which was <br />reviewed by the designated operator and included with the designated operator <br />monthly inspection report. <br />PFJ has reviewed the proper procedure for documenting alarms and maintaining <br />paperwork with both the facility and the designated operator. During the designated <br />operator monthly inspections, the designated operator reviewed copies of work <br />orders, copies of inspection reports by the EHD, alarm logs onsite, and/or <br />construction notes, and having observed the sumps and sensors to be in good <br />condition, concluded that there were no follow up actions required by PFJ at these <br />times. Although there were several alarms that were not included in the designated <br />operator monthly inspection reports for this facility, it is PFJ's position that all alarms <br />were monitored and responded to appropriately, and that PFJ has consistently <br />operated this facility in compliance with the Permit to Operate and other regulations <br />as required by the EHD.
