Please find <br />below a list of the violations noted at the referenced facility and Pilot Flying J's (PFJ) responses. <br />UST Program Inspection <br />Secondary containment repairs completed and tested February 17, 2012, and March <br />6, 2012, required six month follow up testing. This testing has not been completed. <br />All secondary containment components which were replaced require this testing. <br />According to EHD records, secondary containment components were replaced on <br />the T-5, 91 octane turbine sump and the T-1 diesel product line. Secondary <br />containment testing is required six months after installation of new secondary <br />containment components and then every 36 months. Immediately schedule this <br />testing and provide 48 hours notification to the San Joaquin County Environmental <br />Health Department. <br />As noted on the UST Inspection report, Jones Covey conducted retesting at this <br />facility on February 17 and March 6, 2012, following scheduled compliance testing <br />completed on July 27, 2011. The repair permit application was submitted by Jones <br />Covey on July 29, 2011, and approved on December 20, 2011. The permit provided <br />for secondary containment repairs, but did not require a six-month follow up test for <br />these components. A UST Service Request Inspection report implying the follow up <br />testing requirement for the T-1 diesel piping was left with a Jones Covey tech on <br />February 17, 2012, although a copy was not received by PFJ or the Jones Covey <br />main office. This report was provided by the EHD to PFJ following this Notice of <br />Violation, although the due date for testing was listed as July 17, 2012, five months <br />after repairs were made, not six.
