Regarding the question below as to the date of the Return to Compliance
Certification, I have attached the document signed by Joey Cupp on August 20th. I didn't know if I should write
anything on the form noting that we provided a response and backup documentation within the 30 days allowed in
the NTC, but let me know if I need to do anything different. I'm sorry—I can't remember if you need the original
submitted.
Thank you for your help with this and have a good weekend!
Holly Marlowe
Environmental Compliance Specialist
Pilot Travel Centers LLC
Office (865) 474-2421.
Fax (865) 297-1486
From: Holly Marlowe
Sent: Friday, August 22, 2014 1:17 PM
To: 'Elena Manzo (emanzo@sjcehd.com)'
Subject: RE: Return to Compliance-Flying J #618
Importance: High
Hello Ms. Manzo,
I have attached the'completed recordable release statement related to the product found in the bio shed sump at
this site, including information related to the repair of the bio system. A copy has also been sent to the store to
put with other UST records kept onsite. A blank copy of this form is being sent to the stores in San Joaquin
County and will be utilized in the event of an unauthorized release.
Following our discussion this week, I spoke with Aric Morton (Broadbent and Associates) about his call to report
the liquid found in this sump. Although the 2 cups of product was discovered by Mr. Wong, we asked Broadbent
to contact the EHD on May 16th to ensure that the unauthorized release was reported. He told me that he called
(209)468-3420, although he doesn't recall who he spoke with. This person told him that the EHD was already
aware of the incident at this site, so he wasn't given any tracking information.
The next issue discussed was the L6 fuel alarm from 2/27/14. This alarm wasn't logged by the facility when it
occurred and the Designated Operator failed to note this omission on the monthly inspection report in March
2014. Although it wasn't logged by store personnel, the Helpdesk contacted the store to verify if this was caused
by water or fuel. The alarm was determined to be caused by water and cleared after the water was removed.
Typically when an alarm is caused by water, we don't have to dispatch a tech, and therefore the alarm response
log is the only backup available. In this case, while we didn't have to dispatch a fuel tech to resolve this alarm,
there was a crew from Jones Covey onsite working on an unrelated project. They don't leave daily paperwork
onsite for projects, but I was able to get a copy of the daily ticket that referenced water in the T-4 sump. Given
that this is an internal document for Jones Covey, please forgive the typos.
9/26/2014
